A case of alcoholic cirrhosis (hepatology )

A 46-year-old male who is a known case of liver cirrhosis presented to the casualty ward with the chief complaints of Grade IV shortness of breath, yellowish discoloration of eyes, bilateral pedal edema, and abdominal pain for the past 10 days
HISTORY OF PRESENTING ILLNESS
the patient is a known case of liver cirrhosis but gradually 14 days back started developing fever and 10 days back complained with abdominal distension and shortness of breath insidious and slow
PAST HISTORY
Known case of liver cirrhosis had similar episode one yr and also he is type 2 diabetic and is on oral glycemic drugs like grime star.
all the vitals are normal and physical examination showed icterus and bilateral pitting type edema .and dyspnea
the various investigations are done and the provisional diagnosis is given as jaundice under evaluation in case of an exacerbated alcoholic liver cirrhosis

- Fluid restriction (2L/day)

- INJ. PAN 40mg IV OD
- SYRUP LACTULOSE 10ml PO BD
- TAB UDILIV 300mg PO BD (Ursodeoxycholic acid)
- TAB LASI LACTONE 20/50mg OP BD (Furosemide)
- INJ HUMAN ACTRAPID INSULIN SC
- GRBS Charting 6th hourly
- BP Charting 8th hourly
- Monitor Vitals
- TAB DOLO 650mg PO SOS
