the evolution of symptomatology was explained in a detailed manner, with all the possible explanations and flowcharts, and also an account was given on placebo effect. the mechanism of drugs was also discussed thoroughly.

A CHRONIC ALCOHOLIC PATIENT WITH DEVELOPED SEIZURES FOLLOWING CESSATION OF ALCOHOL FOR 24 HRS 

insights - 

the long-term effects of alcohol on neurotransmitters were explained in detail, investigations were done and treatment was given accordingly.


3) Cardiology

the patient is asked a thorough history .also the various investigations are supported by the image and clips . the patient is well taken care of by prophylaxis measures, etc

Question 3 and 4 

A case of alcoholic cirrhosis (hepatology )

A 46-year-old male who is a known case of liver cirrhosis presented to the casualty ward with the chief complaints of Grade IV shortness of breath, yellowish discoloration of eyes, bilateral pedal edema, and abdominal pain for the past 10 days

HISTORY OF PRESENTING ILLNESS 

the patient is a known case of liver cirrhosis but gradually 14 days back started developing fever and 10 days back complained with abdominal distension and  shortness of breath insidious and slow 

PAST HISTORY 

Known case of liver cirrhosis had similar episode one yr and also he is type 2 diabetic and is on oral glycemic drugs like grime star.

all the vitals are normal and physical examination showed icterus and bilateral pitting type edema .and dyspnea 

the various investigations are done and the provisional diagnosis is given as jaundice under evaluation in case of an exacerbated alcoholic liver cirrhosis

  • Fluid restriction (2L/day)
  • alt restriction (2g/day)
  • INJ. PAN 40mg IV OD
  • SYRUP LACTULOSE 10ml PO BD
  • TAB UDILIV 300mg PO BD (Ursodeoxycholic acid)
  • TAB LASI LACTONE 20/50mg OP BD (Furosemide)
  • INJ HUMAN ACTRAPID INSULIN SC
  • GRBS Charting 6th hourly
  • BP Charting 8th hourly
  • Monitor Vitals
  • TAB DOLO 650mg PO SOS


QUESTION


Due to this pandemic, we are unable to appear directly and experience the patient but even though we had a very good experience which is not had been possible without the general medicine department. Dr. Rakesh Biswas sir, HOD made it possible by teaching every single aspect regarding capturing the patient-centered data.

We've got to learn a lot online too. As our college has created a group where we get all the cases taken up by interns and pgs and is being discussed and updated till the patient gets discharged. It feels accomplished .

It's great to have experience of taking the history of the patients admitting to our hospital in the 3rd semester itself and understanding why and how the disease is being caused. 

These questions are making us curious to learn clinical subjects more efficiently.

 We have learned how to take the history and data from a patient to communicate and diagnose the problem.

This paved a way for learning new things differently. 

Sharing knowledge with our peers and interacting with them regarding the cases has been so helping full to us. 

We have to learn the basic knowledge regarding how to interact with the patient and taking a history from them.

This has been a wonderful opportunity to interact with my peers. 

The E logs which have been kept for us are helpful and made me learn so many new things.
