 A 16 Year old female with bluish discolouration of fingers of upper limb. 

  


Hi, i am E. Tejaswi 8 th Sem Medical Student. This is an online e-log book to discuss our patient's health data shared after taking his/her/guardian's consent. This also reflects patients centered care and online learning portfolio. Cheif complaints 

Bluish discolouration of fingers since one month

History of presenting illness

Patient was apparently a symptomatic one month back. Then she developed bluish discolouration of fingers, which was insidious in onset and asymptomatic. There are no aggravating or relieving factors.

No history of shortness of breath

No history of exposure to chemicals

No history of injury or trauma

No history of use of antibiotics

No history of pesticide exposure

No history of fever

No harmful factories around her college

No history of photo sensitivity

Past history

History if similar complaints in the past 

Not a known case of HTN, DM ,CAD, seizures TB,seizures, asthma 

Personal history  
Appetite normal 
Non veg 
Bowel and bladder 
No known allergies 
No addictions 


Family  history 
Not significant 

General examination 

Patient was conscious coherent, cooperative

Not adequately built and  nourished

Sign of Pallor present.                  

signs of cyanosis  present  - bluish discolouration of fingers , and  lips dark in colour . 

No signs of clubbing , lymphadenopathy , oedema 


Systemic examination


CVS- S1, S2 heard , no murmers 
RS - BLAE + , NVBS
P/A- Soft , NT
CNS - No FND

58 yr old with stroke

58 year old male construction worker by occupational came with chief complaints of Burning sensation at right upper half of the body since 2years History of present illness Patient was apparently asymptomatic 2years than one night he suddenly  complained  of loss of vision in both eyes and he developed weakness of right upper limb and lower limb and deviation of mouth to left side which is sudden in onset preceeded by 30-40 episodes of vomiting non-bilious , non-blood tinged, projectile which was associated with involuntary movements of both upper limbs and lower limbs , associated with frothing , tongue bite , outward fixation of left eye ball.
