Hi, i am tejaswi 8thSem Medical Student. This is an online e-log book to discuss our patient's health data shared after taking his/her/guardian's consent. This also reflects patients centered care and online learning portfolio. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence based inputs. Hope this will be informative!

A 65 yr old male patient a resident of choutuppal came with cheif complaints of fever since 4 days  chest pain since 3 days , shortness of breath since one day  and decreased  urinary output since morning


History of present illness 

Patient was apparently asymptomatic till 4 days ago then he develeoped then he developed fever, which was which was high-grade, insidious in onset, gradually progressive, intermittent, which is not associated with chills and rigorand  is relieved on medication. he also complains of shortness of breath grade 4 associated with right side, localised chest pain. He also complains of decreased urinary output which is relieved on foleys catheterisation. He also complains of productive cough yellowish thick sputum since 4 days non foul  smelling non blood stained . He also complains of tingling sensation in the upper limbs and tingling , burning sensation in the feet . 

No complaints of pain abdomen,  vomitinggg , burning micturition ,loose stools .

History of past illness 
He is a known case of Dm type 2 since 5 yrs . He is on medication .(  on tab gliclazide 80 mg and metformin 500 mg )
Not a k/c/o HTN, CAD, CVA , TB , asthma , epilesy , thyroid disorders .

58 yr old with stroke

58 year old male construction worker by occupational came with chief complaints of Burning sensation at right upper half of the body since 2years History of present illness Patient was apparently asymptomatic 2years than one night he suddenly  complained  of loss of vision in both eyes and he developed weakness of right upper limb and lower limb and deviation of mouth to left side which is sudden in onset preceeded by 30-40 episodes of vomiting non-bilious , non-blood tinged, projectile which was associated with involuntary movements of both upper limbs and lower limbs , associated with frothing , tongue bite , outward fixation of left eye ball. After which patient went into coma for 3 months . He was taken to rk hospital Hyderabad treated there for 15 days than was brought back home .
